· Skin condition: mild skin laxity / skin thickness and elasticity are adequate

· Chief concern: 'The under-eye fat makes me look tired and older.'

* Consultation

· This patient is a woman in her 60s whose primary concerns are under-eye fat puffiness and skin laxity.

· We will create smooth, refreshed under-eyes through fat repositioning combined with excision of the sagging skin.

· Although she is in her 60s, her skin elasticity is fairly good, so the risk of ectropion (lower eyelid eversion) is relatively low. That said, overly aggressive skin excision must absolutely be avoided!

· The under-eye area looks smooth and clean. The incision line has healed beautifully — the scar is barely noticeable :)

* Key Points

Attempting to resolve sagging under-eye skin by excising too much can cause ectropion (lower eyelid eversion). Not only does this look unpleasant, but it also causes significant eye dryness and discomfort. Never try to remove too much skin at once!
