Incision Ptosis Correction + Medial Epicanthoplasty + Lateral Canthoplasty (Revision)

A teenage patient's revision eye surgery combining incision ptosis correction, medial epicanthoplasty (left side only), and lateral canthoplasty after a prior procedure left significant scarring and crease failure.

* Basic Information

· Female, teens

· History: partial incision double eyelid surgery + medial epicanthoplasty approximately 1 year ago (patient was unsure of the exact procedure)

· Levator function: noticeably weak

· Excess skin: approximately 1 mm

· Skin thickness: slightly thick

· Patient's requests: 'The crease seems to have come undone not long after surgery. I want a clearly defined crease that looks connected all the way from the inner to the outer corner. I also dislike how thick the inner crease looks, and the medial epicanthoplasty scar is very prominent.'

* Consultation

1. Significant scarring remains from the previous surgery — especially at the medial epicanthoplasty site — and the degree of epicanthal fold release is notably asymmetric between the two sides. The double eyelid crease (created via partial incision, as the patient recalls) also came completely undone shortly after surgery.

2. While the existing scars cannot be fully eliminated and there is limited excess skin, I will lower the starting point of the crease slightly to create an in-and-out appearance.

3. Medial epicanthoplasty will be performed on the left side only to soften the sharp inner corner shape and improve bilateral symmetry to a reasonable degree.

4. We also discussed lateral canthoplasty, as the eyes appear sharp and the outer corners look narrow. However, given the patient's inherently small bone structure, the extent of correction will be limited.

· Although it has only been one month and some swelling remains, the patient is very pleased with the significant improvement in overall appearance.

· The pre-existing scars could not be completely eliminated; however, by rolling the crease inward, the inner corner was made to appear narrower and more refined.

· Due to the patient's small bone structure and deep-set eye shape, only a minimal lateral canthoplasty was performed — a slight disappointment on our end.

· Revision surgery always involves many variables, and effectively managing pre-existing scar tissue is the central challenge of such procedures.

· As the swelling continues to subside, an even more natural result is expected.
